The following forms must be returned to the Department of Educational Psychology by December 15th at the latest (IDET accepts these materials until April 1st).

The following must be completed online no later than December 15th (unless you are applying to the IDET program).

NOTE: The department requires materials other than the above-linked forms. If you feel you have completed the application process please call us at (801) 581-7148 to verify we have received all of the necessary paperwork from you.
